Delaware Health Study

By participating, your household is doing a valuable public service, helping to guide public health policies and prevention programs across the state.

As noted in the letter your household received, the survey is designed to be completed by a randomly selected adult in each household. Please ask the member of your household who is 18 years of age or older, AND has the next birthday to complete the survey.

The adult who completes the survey will receive a $10 Amazon gift code immediately after completing and submitting the questionnaire, in appreciation for their participation.
