1) Who is the sponsor of the study? Who gets this information?
The Delaware Tobacco Attitudes and Media study is sponsored by the Delaware Department of Health and Social Services.
2) Why is this survey being conducted? What is the purpose of the study?
The Tobacco Attitudes and Media study is conducted to collect information on health and tobacco from both tobacco users and non-users. Information gathered from the study will help the Delaware Health and Social Services better understand health issues and plan health programs.
The study was created to assess the prevalence of tobacco use, as well as the factors promoting and impeding tobacco use among adults.
3) What is the survey about?
This survey asks some general questions about your health. It also asks about your knowledge, opinions, and behaviors related to tobacco use. If there are questions you wish not to answer, you can refuse an individual question and still participate in the study as a whole.
4) How did you get my address?
Your household was randomly selected from a list of residential addresses in Delaware. Your participation will help represent your community.
5) What is the information used for?
The information collected in this survey helps inform Delaware programs and policy. Tobacco prevention program administrators, lawmakers, researchers, and the press use the data. Most people do find the survey interesting and no identifying information will be connected to the responses you provide.
6) Do I have to participate?
You were randomly selected to represent thousands of adults. We need to include people with all types of behaviors and attitudes towards tobacco use. This is your opportunity to tell us your attitudes and opinions towards tobacco use even if you do not smoke or haven't smoked for years.
The Delaware Department of Health and Social Services is interested in your responses, even if you don't smoke or haven't smoked for years.
The first sections cover tobacco use, before moving on to other related topics.
7) Who should complete the survey?
The person in your household who is 18 years or older with the next birthday has been selected to complete this survey. This is a method of random selection and is important to make sure that the experiences of all types of people are represented.
Eligible household members include all adults (aged 18 years or older), who consider the household their home, even though they may not be home at the time of the call. These do not need to be related family members, and may include roomers and live-in domestic workers.
Household members do not include adult family members who are currently living elsewhere (at college, a military base, a nursing home, a correctional facility, etc.).
8) How long will the survey take?
On average it should take approximately 15 minutes to complete.
9) Whom should I contact with questions?
